Stockton Insurance Enrollment

To enroll in the University's major medical plan, provided by Aetna Student Health, please complete the online enrollment process as detailed below. If you do not complete the enrollment or submit a waiver by the posted deadline you will be automatically enrolled in the plan; you will not receive an insurance card or show eligibility in the Aetna system until after the waiver deadline.

Your premium is billed to your student account. For detailed information regarding the benefits and exclusions of the plan, read the plan brochure. Please remember that this is an accident and illness plan. Most preventive care is only available at the Cowell Wellness Center, which requires you to pay $120 per semester to be seen. The only time the plan will be pro-rated is for a qualifying life event.

Enrollment Instructions:

1. Visit the Pacific page at Aetna Student Health
2. Click Plans & Products Offered to You on the left hand navigation pane
3. Click on the Enroll/Waive link in the left hand navigation pane
4. Select the link titled "Waive or enroll online for your University of the Pacific plan."
5. Follow all of the instructions to complete enrollment.

Plan Summary 2009-2010 school year:

• $589 per semester
• 100% coverage for most services at the Cowell Wellness Center once the health fee has been paid with a $10 co-pay for CWC dispensed prescriptions
• 80% in-network coverage for most accident or illness related claims after a $400yearly deductible when services are rendered outside of the CWC
• 100% in-network coverage for ER visits after the $400 yearly deductible is met
• 20% co-insurance for prescriptions outside of the CWC
