Cowell Health Requirements

Physical

You must present evidence of a physical. The physical may performed by a Physician, Nurse Practitioner, or Physician’s Assistant. The form must be signed and dated by the provider.

Tuberculosis Clearance

If you have no previous history of a positive Tuberculosis (TB) skin test, you must receive a Tuberculin skin test (PPD) within three months of enrollment. For those who have had a positive TB skin test, an annual chest X-ray is required. You may obtain a PPD from your own physician, a public health agency, or at the CWC.

Immunizations

The following immunizations are required:

  • Measles/Mumps/Rubella (MMR): completion of 2 dose series, one month apart
  • Tetanus/Diphtheria (Td): within the past 10 years
  • Hepatitis B: completion of 3 dose series, 0,1, and 6 months apart; minimum of one dose if not previously started
  • Read information sheet on Meningitis Vaccine, sign, and return to Cowell Wellness Center

The following immunizations are not required, but recommended:

  • Meningitis vaccine, if you are a freshman and plan to live in a residential hall
  • Varicella (chicken pox) vaccine, if you have never had the disease
  • Influenza vaccine
  • Human Papillomavirus (HPV) vaccine (for females only)

If you have a medical exemption for any of the required immunizations, you must contact the Cowell Wellness Center for a medical waiver. We provide the above immunizations excluding the varicella vaccine.

Health Insurance

The University requires that all students enrolled in 9 or more units have health insurance. Insurance helps cover the cost of care which cannot be provided by the Cowell Wellness Center, especially in an emergency or any situation requiring hospitalization.
